Skin Tag Removal
Skin tag removal and seborrheic keratosis removal are considered cosmetic procedures. Most insurance carriers do not pay for cosmetic services, thus payment for these procedures is expected in full at the time of service.
Cosmetic Procedure Policy
We are pleased to offer a complete range of cosmetic services, including Velasmooth cellulite treatments, IPL hair removal, photo rejuvenation, acne treatments, chemical peels, BOTOX, Restylane/Perlane/Juvederm/Voluma/Silk/Sculptra, and Sclerotherapy. These services are not covered by insurance and payment for these procedures is expected in full at the time of service.
Insurance Referral Policy
If your insurance carrier requires a referral from your primary care physician, you are expected to bring a valid referral form, signed by your primary care physician, to your appointment and to pay the co-payment amount that is listed for a specialist provider. If you do not have a valid referral in hand (or a faxed referrals), specifying all services requested by your primary care physician, you will be asked to reschedule your appointment. In addition, if you present without an insurance card for your appointment you will be asked to reschedule your appointment.
